Covering everything can be very, very pricey – anywhere from $250 to $400 a month, if you’re perfectly healthy and don’t need maternity.

Health insurance does not cover all of that. No health insurance covers all of that.
If you need insurance to cover all of that, then you need health insurance, dental insurance, and vision insurance. Those are three separate things. You can buy them from the same company, if you want, but there is no real need to do so. It may be worth checking if you can get something better by buying them from separate companies.
Health insurance covers medical (doctors, emergency room, surgery, hospital, etc.), but not the dental and vision stuff. For dental and vision, you need dental insurance and vision insurance.
You want coverage that has all the pops and whistles. To get that you will pay through the nose. You’re probably better off just paying for vision and dental costs yourself. It will probably be less out of pocket than the sky high premium you’d be paying.
As to which companies offer what, it depends on your state. Talk to several local agents and get quotes from a variety of companies.
None. Health insurance doesn’t cover dental. You’re talking about different policies. Also, vision is best served out of pocket.
Contact a good health insurance broker and they’ll help you find a plan. They’re free to use and get paid by the company, but it in NO WAY changes what you would pay if you bought the same policy elsewhere. Insurance is a regulated business.
Major insurance companies sell all of those types of plans. When you buy individual insurance as opposed to group insurance at work these are all separate. You can call an insurance agent and find out what insurance carriers have which plans in your state.
If you are under 26 you can be added to your parents insurance.
